Every dental residency program requires the submission of a dean’s letter with your admissions application. Also known as an Institutional Evaluation Letter Form (IEF), the dean’s letter functions as recommendation letter from your time as a student at Pitt Dental Medicine. Features of the letter include your final GPA and class rank, didactic and clinical faculty feedback, academic integrity violations, professionalism/ethics violations, code of conduct violations, various awards and honors you may have received, and highlights of your involvement in student organizations at Pitt Dental Medicine.
Students and alumni who are requesting an IEF are required to submit the following to the Office of Student Affairs to process your IEF.
Your up-to-date CV
- D3/D4s will submit in your Class Canvas Assignment
- Alumni will email a copy to dentalstudentaffairs@pitt.edu
Signed Release for Letter of Recommendation (FERPA Waiver) DocuSign
- Alumni and Students: Sign the FERPA Waiver at this website.
Complete the following for the programs to which you are applying
- Email the Office of Student Affairs a Contact List of non ADEA PASS programs you are applying to. Please include the Program Name and contact email in your request to dentalstudentaffairs@pitt.edu.
- Request an IEF in ADEA PASS. Please see instructions at the ADEA Institutional Evaluation web page.
Incomplete IEF requests will result in a letter not being sent. All requests must include a signed FERPA waiver or the School will decline your IEF request.
Please send all documents at minimum 30 days prior to the first deadline (or whenever you need your letter). Please note that it can take a minimum of 4-6 weeks for your letter to be received. Requests may be prioritized based on time of submission and first application deadline.
Any questions you have can be answered by the Student Services Administrators in the Office of Student Affairs.
