Professional Referrals
Treatment for oral and maxillofacial surgery at Pitt Dental Medicine is only by referral.
The referring provider (dentist, orthodontist, PCP, specialist doctor) should write a letter of referral to the appropriate patient coordinate from the list below. Letters may be faxed or may be mailed to:

Please include the nature of the patient’s problem and their contact information, along with a radiograph (X-ray) of affected area. Upon receipt of this information, the Oral and Maxillofacial Surgery Specialty Practice will contact the patient and arrange an appointment. We will not be able to schedule a patient appointment without a letter of referral.

Services Provided by the OMS Specialty Clinic
The OMS Specialty Practice provides the full spectrum of surgical care, including bone grafting, dental implants, extractions (simple, surgical, wisdom teeth),facial trauma, jaw surgery (orthognathic), and TMJ, among other treatments. We also work cooperatively with the other Pitt Dental Medicine Specialty Clinics and can internally refer you to ensure you receive all of the care your dentist recommends.
What to Bring to Your First Appointment
- Plan to arrive 15 minutes earlier than your scheduled appointment to complete your medical history. Upon arrival, please check in with our Front Desk Coordinator.
- Have available your medical and dental insurance cards. If you do not have a dental insurance card, please have a copy of this information. NOTE: Any co-pay noted on the medical card to see a specialist is due at the time of the appointment.
